Activity Report Form

Thank you for being a servant leader in our club! The purpose of this form is to let everyone know about the wonderful activities we do. This information will be sent to our secretary to consolidate the information that will be sent to Kiwanis International in the monthly report form on the 10th of each month. Please submit as soon as the activity is completed. This will help you remember all the details and ensure that our secretary is not inundated with reports at the last minute. For activities to be included in the monthly report please be sure they are submitted by the 5th of each month. Thanks!
